In keeping with the vision of St Agnes’ Parish at Centacare, our mission is to promote an inclusive and supportive community and provide high quality, accessible service to empower individuals, couples and families to reach their full potential.
Centacare is an approved NDIS participant under the name of: Catholic Parish of St Agnes Port Macquarie (Registration Number: 29709979). To access support under the NDIS you must first register through the My Place NDIS Participant Portal at: ndis.gov.au.
While particular programs are designed for specific groups, Centacare’s services are available to all regardless of race, religion or economic status.
Established in 1995, Centacare is a not-for-profit service with offices in Port Macquarie and Kempsey providing outreach to Port Macquarie, Kempsey, Wauchope, Laurieton, Taree, Nambucca Heads and Macksville.

Lifetime Care and Support
Centacare offers a Lifetime Care approved case management service for the treatment, rehabilitation and care of people who have been severely injured in a NSW motor accident and are a participant of the Lifetime Care and Support Scheme.
IMAGE provides supported accommodation for adults with intellectual and physical disabilities that have medium to high support needs.
