At St Agnes’, we have walked alongside people with disabilities and their families for over 30 years, providing personalised support that respects independence, goals and preferences.

Whether you need support at home, in the community, or navigating services, we’re here to make it simpler, safer and more meaningful.

Our disability services

Our disability services are designed uniquely around you. We take time to understand what matters most, then build support that fits your life. 

Coordination of support

Everyone's NDIS journey is different, but you don't have to navigate it alone.

Our team works alongside you to help you understand your plan, connect with the right supports, and make the most of your funding.

We take the time to get to know your goals, needs and preferences, so we can help you find services that are the best fit for you.

We can support you to:

  • Understand and use your NDIS plan
  • Connect with suitable providers and services
  • Coordinate and manage your supports
  • Navigate changes in your circumstances
  • Prepare for plan reviews and reassessments

Community and in-home support

We provide support that helps you live comfortably at home and stay connected to your community.

Our practical, personalised support tailored to your daily life and goals includes help with personal care, meal preparation, household tasks, getting to appointments, and more.

We also support you to get out and about, whether that is attending social activities, joining community groups, or simply enjoying time outside the home.

Our focus is on building your independence, maintaining routines, and supporting what matters most to you with consistent, caring staff you can trust

Day centre disability support services

We provide welcoming place to learn, connect, and enjoy meaningful activities.

Our day centre in Kempsey is open Monday to Friday and offers a variety of engaging programs tailored to individual goals and interests. Activities include cooking lessons, drumming, fishing, sports, and social events.

We focus on building confidence, independence, and social connections in a safe and supportive environment.

Supported independent living 

We provide safe and supportive 24/7 housing for greater independence.

Our supported independent living program provides round-the-clock support in comfortable homes. Staff assist with daily living tasks such as meal preparation, personal care, household routines, and medication management.

We focus on helping residents develop life skills, build confidence, and live as independently as possible while ensuring safety and wellbeing at all times.

Clients are supported to pursue personal goals, social activities, and community participation.

Our team works closely with each individual to create a personalised support plan that balances independence, choice, and consistent care.

NDIS Code of Conduct 

We uphold the rights of people with disability, including the right to dignity, respect, and to live free from abuse, exploitation, and violence. All our workers must follow the NDIS Code of Conduct. This means your support worker will:

  • Treat you with respect and give you freedom to make your own decisions
  • Respect your privacy
  • Be competent, skilled, and professional
  • Act with integrity and honesty
  • Ensure you are safe and respond quickly to risks or incidents
  • Help to prevent violence, abuse, or neglect
